Policy/Waiver/Liability Policies READ CANCELLATION RULE CANCELLATION/RESCHEDULE/NO-SHOW: You are able to reschedule/cancel your appointment 24 hours before your appointment. If you cancel or reschedule in less than 24 hours, the card that you booked with will be charged HALF OF YOUR TOTAL OF THE SERVICE BOOKED (for example.. your service is $100 you will be charged $50 for canceling/rescheduling outside of the 24hr window.) If you no show I will charge the card on file the same fee 50% of your booked service. •Come with a clean face and clean lashes. I will charge you a $10 lash bath fee if I have to clean them for you. •Fill-ins I require that you have at least 40% of your lashes remaining in order for it to be considered a fill. If you do not have 40% of lashes I have to charge you the full set fee. Lash fills are required every 2-3 weeks according to your lash shed cycle/after care. LATE POLICY: You can not be more than 10 minutes late. If you do not contact me 10 minutes after your appointment time, you are considered a no show. If you are 10 minutes late, you will be charged a $15 late fee. WAIVER/LIABILITY Although every precaution will be taken to ensure your safety and well being before, during and after your lash extension application, please be aware of the following information and possible risks. I understand that some irritation, itching or burning may occur on the skin if the bonding agent comes into contact with it. I understand that this is a semi-permanent procedure, as my natural lashes will continue to grow and fall out normally, making touch-up or additional appointments necessary to maintain the original look achieved by replacing the lashes that have fallen out. Most clients require a "fill" appointment every 2-3 weeks. I understand that pictures may be taken before, during, and after the appointment. If you do not agree to this term, please contact me before booking. I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially envisioned due to my eye shape, quantity of my lashes, etc.. I understand that additional conditions could occur or be discovered during the procedure which could affect my ability to tolerate the procedure. I consent to “before and after” photographs for the purpose of documentation, potential advertising and promotional purposes. I understand I have up to 24 hours after my service to contact Miss Cherry in case I have lash retention issues to receive a fix free of charge. Passed 2 days of my service, they can be fixed at a discounted fee. Unfortunately I do not offer refunds. But I will offer a solution. I understand that if I have any concerns, I will address these with my lash extension specialist. I give permission to my lash extension specialist to perform the lash extension procedure we have discussed, and will hold her harmless and nameless from any liability that may result from this treatment. I have accurately answered the questions above, including all known allergies, prescription drugs, or products I am currently ingesting or using topically. I understand my lash extension specialist will take every precaution to minimize or eliminate negative reactions as much as possible. In the event that I may have additional questions or concerns regarding my treatment, I will consult the lash extension specialist immediately. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I CERTIFY that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the lash extension specialist, whose signature appears below, responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed at the time of booking.
